The prognosis of colorectal cancer is related to the depth of tumour invasion. The aim of this study was to compare a clinical score, a computed tomography (CT)-based score and a dynamic magnetic resonance imaging (MRI) score in predicting tumour invasion of the rectum and sigmoid colon. Patients who underwent elective colorectal resection were assessed prospectively. Depth of invasion was assessed using MRI, rectoscopy and colonoscopy. A score was calculated from the number of lesions measured with these three techniques. The CT score was based on the size of the tumour and the presence of nodal metastases. A total of 41 patients was included in the study. The dynamic MRI score was the best method to predict depth of invasion. The accuracy of this score was similar to that of the rectoscopy and colonoscopy scores (area under the curve (AUC), 0.94 versus 0.83 and 0.76, respectively).
